These forms are to provide the Jonesboro 911 Communications Center information that will be useful in a variety of situations ranging from a missing autistic child to livestock in the roadways to locating victims of a natural disaster that may be in a storm shelter. This information will be considered confidential and will not be shared with anyone outside of public safety personnel. NOTE: Information collected on these forms is NOT for requesting emergency or non-emergency services. Police, Fire and/or Emergency Medical Services will not be dispatched as a result of information collected.
